Jamie Oliver calls for more funding to back school meals revolution

Jamie Oliver is urging ministers to put more money into school meals as official figures show growing numbers of pupils are eating the healthy dinners his campaign inspired.

The benefits to children's health should prompt more government investment, said Oliver. Short-term financial pressures should not threaten the provision of nutritious school food, he added.

He spoke out after data from the government's School Food Trust showed the number of pupils in England eating a hot lunch at school had risen by 320,000 in the past year – an increase for the third consecutive year.

The figures contradicted health secretary Andrew Lansley's assertion last week that take-up had fallen since the quality of school meals was radically overhauled after Oliver's Jamie's School Dinners series in 2005. The programme revealed that many children were being fed chips and Turkey Twizzlers.

"Some people in government might look at the figures and think that it's now time to take the foot off the gas because it's a success story. That would be completely wrong," said Oliver. "Now is the time to move up a gear. This is the time for education and health departments to invest in those schools who still have problems with lack of training for dinner ladies or who suffer from having a dining area that's too small or uninviting."

He asked ministers not to let the progress made in pupils' eating habits be lost. "Investment now saves lives and [the] NHS billions in the future. We're on the right track with school meals. We can't allow anything to slow this down," he said.

But the Department for Education has refused to promise to extend the £80m-a-year subsidy, the school lunch grant, it gives local councils to help them provide the healthy school meals Labour introduced. It is guaranteed until the end of March 2011.

"All future spending decisions for after 2011 will be part of the comprehensive spending review in the autumn and we can't pre-empt the decisions or content," a spokeswoman said.

"Ministers want school in food to remain healthy and will set out decisions in due course."

The Liberal Democrat children and families minister, Sarah Teather, added: "We welcome the increase in the number of children getting a healthy meal in schools. We want to ensure school meals continue to be healthy and will set out the next steps for school food policy in due course."

The Children's Food Campaign wais concerned that widespread cost-cutting in Whitehall could risk future funding, making it difficult for councils to produce healthy meals at affordable prices.

The National Obesity Forum spokesman, Tam Fry, said Lansley should stop denigrating Oliver's campaign.

School meals in England are backed by legal standards requiring them to include a certain amount of carbohydrates, vitamins and minerals, and restricting the amount of fat, salt, sugar and saturated fat they can contain.

A Department of Health spokeswoman said that contrary to media reports, the health secretary had applauded Oliver's campaign when he addressed the British Medical Association's annual conference. He criticised Labour government restrictions on the campaign.

    

Causes Of Skin Cancer

Ultraviolet (UV) light exposure, most commonly from sunlight, is overwhelmingly the most frequent cause of skin cancer.
Other important causes of skin cancer include the following:
  • Use of tanning booths
  • Immunosuppression-impairment of the immune system, which protects the body from foreign entities, such as germs or substances that cause an allergic reaction. This may occur as a consequence of some diseases or can be due to medications prescribed to combat autoimmune diseases or prevent organ transplant rejection.
  • Exposure to unusually high levels of x-rays
  • Contact with certain chemicals-arsenic (miners, sheep shearers, and farmers), hydrocarbons in tar, oils, and soot (may cause squamous cell carcinoma)
The following people are at the greatest risk:
  • People with fair skin, especially types that freckle, sunburn easily, or become painful in the sun
  • People with light (blond or red) hair and blue or green eyes
  • Those with certain genetic disorders that deplete skin pigment such as albinism, xeroderma pigmentosum
  • People who have already been treated for skin cancer
  • People with numerous moles, unusual moles, or large moles that were present at birth
  • People with close family members who have developed skin cancer
  • People who had at least one severe sunburn early in life
Basal cell carcinomas and squamous cell carcinomas are more common in older people. Melanomas are more common in younger people. For example, melanoma is the most common cancer in people 25-29 years of age.

Skin Cancer

What is skin cancer?

Skin cancers are the most common type of cancer in white populations, both in the UK and worldwide. Over the last 25 years, rates of melanoma skin cancer in Britain have risen faster than any other common cancer. Most are easy to treat and pose only a small threat to life, but one type, melanoma, is difficult to treat unless detected early.

Symptoms of skin cancer

There are three principal types of skin cancer, and different cancers can have different symptoms and appearances.

The most common is ‘basal cell carcinoma’, affecting a type of cell within the top layer of skin. It’s a slow growing cancer, and doesn’t usually spread to other parts of the body.

BCC affects all sun exposed areas of the body and the main symptom is a small painless lump, pink/brownish-grey in colour, with a smooth surface with blood vessels and a waxy or pearl-like border. The lump grows, developing a central depression with rolled edges.

The second type, involving another type of cell in the top layer, is called ‘squamous cell carcinoma’, and again is not known for its aggression. SCC usually affects the face. The main symptom is an area of thickened, scaly skin and develops into a painless, hard lump, reddish brown in colour with an irregular edge. The lump becomes a recurring ulcer and doesn’t heal.

These two types are known as non-melanoma skin cancer are usually slow growing and occur on sun exposed areas of the skin and rarely spread.

Melanoma skin cancer can occur anywhere on the body and is more dangerous. It’s related to the common mole and changes in the appearance of moles on your body should be checked by your GP. Malignant melanoma tends to spread much more rapidly through the bloodstream than the other two types of skin cancer. It affects the cells which produce the skin's colouring, and if not treated successfully can spread to affect the liver, lungs or brain. The main symptom is a quick-growing, irregular, dark-coloured spot on previously normal skin or in an existing mole that changes size, colour, develops irregular edges, bleeds, itches, crusts or reddens. If an adult has a growing, changing, brown or black mark which cannot be covered by the blunt end of a pencil, this should be shown to the doctor straight away.

Occasionally, melanoma skin cancer may present with swollen lymph glands or rarely in unusual places including the sole of the foot, mouth or eye. If melanoma is diagnosed, then further tests will be done to see if the cancer has spread beyond the skin to other parts of the body. This may involve taking x-rays and scans to look at the liver, brain and lungs.

To find out if it’s skin cancer, and, if so, what sort of skin cancer it is, a doctor will carry out a biopsy, removing all or part of the suspicious growth which is then analysed.

Skin cancer - causes and risk factors

Although scientists have found that those with lighter skin are far more vulnerable to skin cancers, the main cause of skin cancer is overexposure to the sun’s harmful UV rays. A suntan isn’t healthy; in fact it’s a sign of skin damage. The results of sunbathing are skin that ages more quickly and an increased risk of cancer. It’s thought the UV radiation in sunlight causes subtle cell damage which can lead to cancerous changes.

Non melanoma skins cancers result from prolonged sunlight exposure over many years. The main cause of melanoma skin cancer is exposure to short periods of intense sunlight; the kind of exposure people get on a two-week holiday.

Rates of skin cancer of all sorts are extremely low among dark-skinned people and men are more likely to develop cancers on their neck, shoulders and back, whereas in women the cancer is more likely to appear on the legs and arms.

Skin cancer - treatment and recovery

Diagnosis of skin cancer can usually be made by your GP or hospital specialist by simple skin examination. Sometimes, the skin cancer will need to be removed by a small operation or biopsy; both for treatment and lab testing. Other routine tests, including X-rays and scans aren’t usually required.

Non-melanoma skin cancers are usually treated by a common operation to simply cut out the affected area under local anaesthetic. Another method used on smaller cancers is cryosurgery, in which liquid nitrogen is applied to the tumour to freeze it and kill the cells, which simply shrivel and drop off once warmed up.

Some cases of basal cell carcinoma may be suitable for ‘photodynamic therapy’ - which uses a cream to sensitise the tumour - then exposes it to high intensities of light to destroy it.

In the case of melanoma, if there is a suspicion that the cancer may have spread beyond the skin layer, chemotherapy or biological treatment such as interferon may be given to attempt to eradicate skin cancer cells in other parts of the body.

About 1,800 people die from melanoma skin cancer annually in the UK. Even so, nearly 80 per cent of men and over 90 per cent of women are alive at five years following treatment.
Preventing skin cancer

The best way to prevent skin cancer is to avoid too much time in the sun. You don't have to be sunbathing to get burned. You can get too much sun while walking to the shops, driving a car with the windows down, even under light cloud cover. Time of day and location are important too. The intensity of UV radiation increases during the middle of the day, between April to September, nearer the equator and at higher altitudes.

How to protect yourself and your children:

* Stick to the shade between 11am and 3pm
* Cover up with clothes, a wide brimmed hat and sunglasses
* Apply a high-factor sunscreen (minimum SPF15 and three stars) regularly
* Drink plenty of water to avoid overheating
* Avoid using sun lamps or sunbeds

Watch those moles

Many moles aren't cancerous, but it's vital to keep an eye on any you have. Watch out for moles that change shape or colour, become bigger, itchy or inflamed, or that weep or bleed. If you notice any changes or are worried, get them checked by a doctor.

    

Lung cancer



Lung cancer has always been more common in men, particularly those aged over 40. However, as fewer men smoke now rates have fallen. Meanwhile lung cancer rates increased among women until the 1980s as more took up smoking. Although rates among women have stabilised, the disease now claims more lives than breast cancer.

There are several different types of lung cancer, which are generally divided into:

* Non-small cell lung cancer (NSCLC)
* Small cell lung cancer (SCLC)

Nine out of ten cases of lung cancer are caused by the effects of tobacco smoking - and unfortunately the majority of cases cannot be cured.

Symptoms

The key symptom of lung cancer is a persistent cough that gradually gets worse.

If you have the following, or any other symptoms, you must have them checked by your doctor (but remember, all occur in many conditions other than cancer):

* A persistent cough or change in the nature of a longstanding cough
* Shortness of breath
* Coughing up blood-stained phlegm (sputum); blood is a warning sign that always needs urgent investigation
* Persistent chest pain - a dull ache or sharp pain when you cough or take a deep breath
* Loss of appetite and weight
* A drop in ability to exercise and general fatigue

At present there's no effective screening test for lung cancer. If you have any worrying or new symptoms, tell your GP straight away so you can be investigated promptly or reassured that there's nothing to worry about.

Causes and risk factors

As most lung cancer cases are caused by smoking cigarettes, the risk increases with the number and type of cigarettes smoked. Even passive smoking (inhalation of other people's cigarette smoke) can cause a problem, and the longer period over which the patient smokes, the higher the risks. Only one in ten lung cancers occurs in non-smokers. Pipe and cigar smokers have a lower risk than cigarette smokers, but it's still a far greater risk than that of a non-smoker.

Some people seem to be genetically pre-disposed to developing lung cancer, and medical checks in smokers may in future look for these key genes to work out how likely lung cancer is.

Some rare types of lung cancer aren’t related to smoking. Mesothelioma, which affects the covering of the lung (the pleura), for example, is almost always caused by exposure to asbestos. Other causes include exposure to certain chemicals and substances in the workplace, such as uranium, chromium and nickel. These have all been linked to lung cancer but are very rare. Contact your local environmental health officer if you're concerned.

If a person stops smoking the risk of lung cancer is greatly reduced, so that after about 15 years the chances of developing the disease are similar to that of someone who has never smoked.

Treatment and recovery

Treatment of lung cancer depends on the type of tumour, its location and how far it has spread, as well as the persons general health. A process known as staging is used in each case to asses these factors and recommend appropriate treatment, which may include surgery, chemotherapy and radiotherapy. Lung cancer patients may be referred to special clinics.

There are several tests to diagnose lung cancer. These include:

* CT or MRI scans – these sophisticated scans involve lying still inside a machine as it takes images of the internal organs. The pictures that result can give a very good idea of whether cancer might be present but aren’t absolutely definite as cell samples are needed to make a full diagnosis. Scans are often done in order to guide a biopsy to suspicious areas.
* Bronchoscopy - a thin flexible tube is passed gently through your nose or mouth and into the lung airways. Photographs and cell samples are then taken
* Mediastinoscopy - a small cut is made in through the skin at the base of the neck under general anaesthetic just above the breastbone or on the left side of the chest next to the breastbone. Then a thin flexible telescope (mediastinoscope), often with a grabbing device attached down the airways of your lungs, is then passed into the chest, allowing the doctor to examine the area at the centre of the chest and local lymph nodes a small cut is made.
* Lung biopsy - a needle is passed through the skin into the lung and a sample of cells is taken for examination.

When the tumour is away from the centre of the chest and there's little or no spread (typically in NSCLC), surgery is often used. A small part of the lung may be removed, or the entire lung.

You can breathe normally with only one lung. However, if you had breathing difficulties before the operation you may be more breathless afterwards. Breathing tests will be carried out to help you and your doctor decide on the best course of action.

Chemotherapy and radiotherapy will be considered in many cases. Unfortunately, NSCLC is hard to cure, and in many cases, the treatment given will be to prolong life as far as possible - and relieve symptoms. SCLC is different, in particular, it has a tendency to spread to distant parts of the body at a relatively early stage. Chemotherapy and/or radiotherapy are usually more effective in treating this type because it usually spreads to more than one place by the time it's diagnosed and is less likely to be cured by surgery.

In some cases laser therapy can provide some relief and new treatments are also being studied and starting to come into wider use, including biological therapies which are drugs that block the signals that cancer cells need to grow. In NSCLC, different ways of giving radiotherapy and chemotherapy are being investigated.

Sometimes new symptoms can develop which may be caused by the spread of the cancer to other parts of the body, but could also come about because some lung cancer cells produce hormones that upset the body's chemical balance.

    

Leukaemia


What is leukaemia?

Leukaemia affects white blood cells, which are part of the body's defence against infection. It can develop quickly (acute), or more slowly (chronic).

Depending on the type of white blood cell affected, most acute leukaemia is either acute lymphoblastic leukaemia (ALL) or acute myeloid leukaemia (AML). Chronic leukaemia is usually either chronic lymphocytic leukaemia (CLL) or chronic myeloid leukaemia (CML). However there are also several less common types of leukaemia.

In acute leukaemia, immature white blood cells accumulate in the body and can disrupt the function of many tissues and organs. Cells in chronic leukaemia are slightly abnormal and do not function as well as they should.

Symptoms

Often there are no symptoms at first and leukaemia may be a chance finding on a blood test. When symptoms do appear they may be vague and non-specific, similar to a flu-like illness.

Common symptoms include:

* Generalised weakness and fatigue
* Anaemia
* Frequent infections and fever
* Weight loss
* Excessive bruising or bleeding
* Pain in the bones and joints
* Breathlessness
* Enlarged lymph glands, liver and/or spleen
* Abdominal discomfort

Causes and risk factors

Leukaemia is the ninth most common cancer in men in the UK and the eleventh most common cancer in women in the UK. There are about 7,000 new cases in the UK each year and more men are affected than women.

It is the most common form of childhood cancer, but is ten times more common in adults.

The incidence of acute and chronic leukaemia is about equal. Slightly more men than women develop acute leukaemia, and slightly more men than women develop CML.

The risk of developing acute leukaemia is increased by radiation exposure, smoking, previous cancer and treatment for cancer, and blood disorders that damage the bone marrow. Chemical (benzene) exposure increases the risk of AML, while not smoking reduces it.

The risk of developing CML is increased by ageing, radiation exposure, chemical (benzene) exposure.

The risk of developing CLL is increased by ageing, being male and having a member of the family affected by CLL. Avoiding exposure to benzene reduces the risk of AML and CML.

Treatment and recovery

The diagnosis of a leukaemia will be based on a number of different tests including blood and bone marrow tests, scans to see which organs and tissues may be affected and tests to look for particular chromosomal or genetic changes. These will help to pin down the particular type of leukaemia and the treatment that should be recommended.

Not everyone diagnosed with leukaemia will need treatment immediately, and some patients never require treatment (CLL, for example, can progress so slowly in some older people that they are more likely to die from other problems first). However, for those who do, the earlier it begins the more effective it is.

Treatment for acute leukaemia can include chemotherapy (anti-cancer drugs), steroids, radiotherapy, intensive treatments (including bone marrow or stem cell implant) and growth factors (substances that stimulate the bone marrow to make blood cells). Treatment for chronic leukaemia can also include biological therapy, such as interferon alpha.

Prognosis and survival from leukaemia varies enormously, and depends on factors such as the type of leukaemia you have, how advanced it was when first diagnosed, how well it responds to chemotherapy treatment, and how generally fit you are. Each person should discuss this individually with the team in charge of their care.

Calcium pills 'increase' risk of heart attack


Calcium supplements taken by many older people could be increasing their risk of a heart attack, research shows.

The study, in the British Medical Journal, said people who took supplements were 30% more likely to have a heart attack.

Data from 11 trials also suggested the medicines were not very effective at preventing bone fractures.

Almost 3m people in the UK are thought to have osteoporosis and many take calcium pills to prevent fractures.

The study recommends doctors review their use of calcium supplements for managing osteoporosis.

In all 12,000 people aged over 40 took part in the trials of calcium supplements of 500mg or more a day.

The risk of heart attack was seen across men and women, was independent of age and the type of supplement given.

A small increased risk of death was seen in the study but was not statistically significant, the researchers said.

The reason for the increased risk of heart attack is not clear but it is thought the extra calcium circulating in the blood could lead to a hardening of the arteries.

Calcium in the diet is safe and the Food Standards Agency recommends adults have 700mg of calcium a day from milk, cheese and green, leafy vegetables.

Dr Alison Avenell, from the University of Aberdeen which did the research with colleagues in New Zealand and the US, said the evidence suggests calcium supplements only have a limited benefit in preventing fractures, especially when compared to other treatments available.

"It is a balance of risks - people should consider the risks involved and how they apply to their own circumstances and discuss the matter with their GP," she said.

She added the results did not necessarily apply to younger people with conditions for which they take calcium.

Judy O'Sullivan, senior cardiac nurse at the British Heart Foundation, said the results should be interpreted with caution because the trials did not set out to look at the risk of heart attack.

"However, the research should not be completely ignored," she said.

"Any new guidelines on the prevention of fractures in those most vulnerable to them should take this type of analysis into account."

Dr Claire Bowring, of the National Osteoporosis Society, said: "We've always recommended that people should aim to get the calcium they need from their diet to help build stronger bones.

"If you get all of the calcium that you need from your diet and adequate vitamin D from exposure to sunshine, then a supplement will not be necessary."

She said there were still questions to be answered about the treatment of osteoporosis but advised people taking calcium supplements to talk to their GP, especially if they have a heart condition.

The National Osteoporosis Society said most people should be able to get enough calcium through their diets, rather than reaching for the medicine cabinet.

The researchers said those who had a diet naturally high in calcium were at no increased danger.

Herpes virus used to treat cancer


Doctors say they have used a genetically engineered herpes virus to treat successfully patients with head and neck cancer.

A London hospital trial of 17 patients found that use of the virus alongside chemotherapy and radiotherapy helped kill the tumours in most patients.

It works by getting into cancer cells, killing them from the inside, and also boosting the patient's immune system.

Further trials are planned for later in the year.

Head and neck cancer, which includes cancer of the mouth, tongue and throat, affects up to 8,000 people every year in the UK.

Study leader Dr Kevin Harrington, who is based at the Institute of Cancer Research in London, said current treatments were effective if the cancer was picked up early but that many patients were not diagnosed until it was more advanced.

The herpes virus, which is also being tested in patients with skin cancer, is genetically manipulated so that it grows inside tumour cells but cannot infect normal healthy cells.

Once there it has a triple effect - it multiplies, killing tumour cells as it does so, it is engineered to produce a human protein that activates the immune system and it also makes a viral protein that acts as a red flag to immune cells.

'Potential weapon'

In the 17 patients injected with the virus, in addition to their standard treatment, at the Royal Marsden Hospital, 93% showed no trace of cancer after their tumour had been surgically removed.

More than two years later, 82% of patients had not succumbed to the disease.

Only two of 13 patients given the virus treatment at a high dose relapsed, the journal Clinical Cancer Research reported.

There were no safety concerns with use of the virus, the researchers said, and it is hoped the virus could one day be used to fight other types of cancer.

"Around 35 to 55% of patients given the standard chemotherapy and radiotherapy treatment typically relapse within two years, so these results compare very favourably," said Dr Harrington.

He is now planning a trial comparing the viral treatment with the standard treatment in people newly diagnosed with head and neck cancer.

Dr Alison Ross, senior science information officer at Cancer Research UK, said it would be some time before the treatment could be used in patients as it still needed to be tested directly against standard treatment.

But she added: "This small study highlights the potential of using genetically modified viruses as a weapon to fight cancer."
